Journaling: Diamond often asks patients to keep a headache diary, where parents and kids can document the onset time, severity and duration of pain in hopes of seeing a pattern and identifying the trigger.
Preventative medications: Triptan drugs like Imitrex help stabilize the size of blood vessels if taken at the onset of the headache. But if the child or teen’s migraines are frequent enough (i.e. weekly in occurrence) to impair normal functioning, Sperry said he considers prescribing preventative medications that are used daily — often for a matter of years.
Calcium-channel blockers, some seizure medicines, antidepressants and beta blockers commonly are used, but most of the drug groups include constipation, insomnia or drowsiness side effects, Sperry said. So it’s a matter of weighing the benefits with the drawbacks.
“Lots of times, the migraines are so debilitating that families are willing to try these drugs out. I can’t tell you how delighted they are when this kind of stuff works,” Sperry said. “As much as you don’t want to put your kid on drugs, parents get desperate when all their children want to do is go lay down and crawl in a hole and things have spiraled out of control.”
Biofeedback: The Diamond Headache Clinic uses a non-drug therapy technique for children and adolescents. The method teaches patients to control bodily functions — such as heart rate, blood pressure, body temperature and muscle tension — to train their anatomical response to stress. The clinic conducted research on biofeedback and found 90 percent of kids, who are in the prime of their lives in terms of imagination and visualization abilities, who participated in the therapy showed significant symptom reduction.

Genetics: Younger kids are predisposed if their parents have a migraine condition. If one parent has migraines, there is a 40 percent risk that the child will get them, according to the Diamond Headache Clinic. If both parents experience migraines, the probability of the child getting them rises to 70 percent.
Stress: “Nowadays, society is just way too busy and over-stimulated and stressed. People with sensitive nervous systems are easily affected by anything that causes adrenaline to speed up,” Diamond said. “All the medicine in the world isn’t going to help if you’re still going 100 mph because the body needs a cool-down time so it can detoxify.”
Her clinic sees an influx of adolescent patients at the beginning and end of the academic year, when anxiety over homework and extracurriculars is the greatest. This held true for Jennie Kamrath. Her migraines were not nearly as pronounced on the weekends and during extended breaks from school.
Diet: Certain foods or beverages with caffeine, chocolate, yellow cheeses, nitrates, artificial sweeteners and additives can aggravate the condition. And for many children and adolescents, these processed, sugary items are consumed in large quantities, which has unsettling biochemical consequences, Diamond said.
Also, the timing of meals can play a role. “For some of my kids I take care of, this is a big issue,” Diamond said. “They’re getting up at 6:30, 7, eating, and then eating again at 10:30 if they have an early lunch period. They may not have food again until after 4, and that’s a long time to go without caloric intake.”
Sleep: An irregular pattern or an inadequate number of hours can do a lot of harm for those who experience migraines, Sperry said. Again, anything interrupting the physiological norm will have health ramifications.
Light: Bright or fluorescent lighting is the kiss of death, Jennie Kamrath said.
Odors: Overpowering scents such as cleaning products, perfumes, smoke and chlorine can activate migraines, too, Sperry said.
Hormones: Before puberty, boys and girls experience chronic headaches equally, according to the Diamond Headache Clinic. But after teen girls begin menstruating, they are three times more likely to suffer from them than their male classmates because of the hormonal shifts.

Kids diagnosed with adult condition
Since about third grade, Jennie Kamrath has come home from school every day with a splitting headache. The Downers Grove, Ill., resident, now 17, usually makes a beeline for her bedroom, where she closes the blinds, pulls the covers over her head and tries to sleep off the pain for a couple of hours.
“My forehead and eyes would be throbbing, and I had extreme sensitivity to light and certain smells,” Kamrath said. “For a while, I couldn’t do anything. It affected my school work. I never hung out with friends. I couldn’t even go for a run. I just kind of sat at home and withered away.”
When the pounding episodes worsened in middle school, doctors discovered she was suffering from migraines. Like many parents of young chronic headache and migraine patients, Jeanne Kamrath was surprised by her daughter’s diagnosis.
“I prodded Jennie with questions, and we figured out that her headaches kicked in around 2 p.m., so I always thought it was just the wear and tear of the day. I wasn’t too alarmed,” she said. “I didn’t take them very seriously because you kind of assume younger kids aren’t going to get something that your head tells you is reserved for adults.”
Dr. Merle Diamond, co-director of the world-renowned Diamond Headache Clinic inpatient unit at St. Joseph Hospital in Chicago, said moms and dads frequently write off symptoms as minor sinus pressure because migraines don’t seem plausible for the juvenile set. But according to the clinic, 5 percent of children and adolescents (as opposed to 12 percent of adults) battle chronic headaches and migraines — an incidence rate confirmed by other migraine experts.
“We’ve seen 2-, 4-year-olds come through with migraines, so it’s definitely impacting the young demographic. And it’s tough because kids aren’t able to articulate what’s hurting,” said Diamond, Kamrath’s doctor and a board member for the National Headache Foundation. “It’s such a miserable and oftentimes disabling condition to live with.”
Migraines are vascular in nature, with blood vessels in the brain changing size and altering blood flow when set off by various triggers, said Dr. David Sperry, a pediatric neurologist and migraine specialist with Winfield, Ill.-based Central DuPage Physician Group. This contributes to a swollen feeling in the skull.
Additionally, migraines are characterized by vision disturbances, nausea, dizziness, light and noise sensitivity, neck pain, sinus congestion and lethargy, in addition to the standard-issue head pain, Diamond said.
Episodes can last anywhere from several hours to a few days. The average number of chronic attacks in a given month is two, but 25 percent of sufferers get one a week, Diamond said. And the overall ill feeling and inability to focus causes many to call in sick to work or school.
Jennie Kamrath missed so many classes that she has been enrolled in summer school sessions to catch up and will be graduating from Downers Grove North High School a year later than her class. That’s why doctors like Diamond and Sperry hope to raise public awareness of childhood and adolescent migraines so that families can begin treatment earlier and avoid the life-limiting effects.
“For a long time, those in the medical field and society at large didn’t believe kids couldn’t possibly be wrestling with migraine headaches,” said Sperry, who added that 20 percent to 25 percent of his practice is devoted to young patients exhibiting migraine headache symptoms.
“It was kind of ‘Oh, please!’ with them giving a pat on the head and handing over a Tylenol. And this response prolonged the child’s agony.”
Until Jennie Kamrath found Diamond, who pinpointed her chronic tension-type migraines, she worked with doctors who didn’t know how to tackle her condition and thought she was destined for a life of pain as a recluse. But now, the clinic’s alternative therapy regimen has her feeling better than ever.
“I constantly run into families who are stunned when we come to the conclusion that their child is suffering from migraines, and I do think a lot of these are missed,” Sperry said. “But as they start getting recognized more and more, it’ll help promote migraine management, and we can start improving quality of life.”

Children are just as susceptible to headaches as adults are. When this happens, just get away from the smells and give yourself a chance.
